Can you transport your foster animal to and from the OHS as needed? * Required

Dogs in your home must have DHLPP Distemper, Hepatitis, Leptospirosis, Parvo, Parainfluenza) Bordetella (kennel cough) and rabies vaccines. Cats in your home must have FVRCP (Feline Viral Rhinotracheitis, Calicivirus, Panleukopenia) and rabies vaccines.
What types of animals are you interested in fostering? * Required
Do you have experience administering medication to animals? * Required
Are you comfortable fostering an animal who may have an unknown illness or condition? * Required
Are you comfortable knowing that an animal you are fostering may be humanely euthanized if their condition is beyond the OHS's reosurces to treat? * Required
Are you able to isolate your foster animal from other animals in your home for the duration of their placement? * Required
